当クリニックでの診療やセカンドオピニオンをご希望の方は、かかりつけ医や主治医等の紹介状（診療情報提供書）をご手配の上お申し込みください。

病状の変化を考慮し、紹介状の有効期限は発行日より3か月以内とさせて頂きます。

Accordingly, during the initial swallowing function assessment conducted at the start of care, medical information—such as your past medical history, history of present illness, and medication history—is extremely important.

If you wish to receive care at our clinic or seek a second opinion, please arrange a referral letter (medical information summary) from your primary care physician or attending physician before booking an appointment.

In view of possible changes in your condition, referral letters are considered valid for three months from the date of issue.
